fix: exclude mp4 container from AC3 surround passthrough fallback

AC3 audio in an MP4 container is an invalid codec/container
combination. Restrict the surround passthrough fallback to
containers other than mp4 to avoid producing invalid direct
play profiles.

fix: force AC3 passthrough when 5.1 surround output is active
...
Some devices incorrectly report AC3 passthrough as unsupported via
CanDecodeAudio() even when 5.1 surround output is enabled. Work around
this by assuming AC3 support whenever surround output is active.
EAC3 and DTS passthrough detection is left unchanged — CanDecodeAudio()
correctly reports those on tested hardware.

fix: scope audio passthrough to codecs reported by GetAudioDecodeInfo
...
The previous implementation assumed all passthrough codecs (ac3, eac3, dts)
were supported whenever surround output was active, regardless of the actual
device audio configuration.
Instead, use GetAudioDecodeInfo() to check whether each passthrough codec is
available in the current audio configuration. This respects the user's digital
output selection in Roku system settings — for example, a device configured
for Dolby Digital only will have AC3 but not DD+ or DTS in the decode info.
Note: GetAudioDecodeInfo uses 'DD+' as the key for eac3, not 'eac3'.
